Hundreds of individuals receive services each year and stay on average of five days. In the tradition of Lourdes, nurse practitioners, nurses, mental health professionals and counselors are available to meet the diverse needs of our patients.

Transitions Jail Diversion

In collaboration, Lourdes Transitions program works with local law enforcement groups. The focus is on those who are mentally ill and have committed a low level misdemeanor. This unique program allows people who are acutely mentally ill to receive treatment rather than go to jail.

Cullum House

This 10-bed residential home serves people who need structure and support. The focus is on reducing barriers that may be preventing individuals from living independently. Cullum House is staffed by professionally trained and experienced residential counselors.

Outpatient Programs:

Assessment and Referral

We have licensed and master level mental health professionals to conduct an initial evaluation for appropriate outpatient services and will refer to medication management, therapy, substance use disorder treatment, and or case management.

Medication Management

Individuals, experiencing a variety of mental health disorders, often require assessment and ongoing medication management. We have a combination of psychiatrists, nurse practitioners and registered nurses to monitor potential medication side effects and assess the effectiveness of the medications.

Therapy/Counseling Services

Our experienced mental health professionals provide individual and family counseling. Professionally trained therapists place a strong emphasis on solution focused therapy and education.

Substance Use Disorder Treatment

This program provides assessment, referral, Intensive Outpatient (IOP), Outpatient (OP), and DUI Deferment Assessments for people in need of substance use treatment and may also be experiencing symptoms of other behavioral health disorders. Group and individual services are provided by experienced mental health professionals and licensed substance use disorder staff.

Case Management

Our mental health staff customize services for our patients with chronic mental illness. Most of the services are provided in a community setting. The emphasis is placed on helping you or your loved one live independently in the community. Assist clients with housing, medical appointments, discharge planning, payee assistance, and peer support.

  • We also provide case management services to individuals being released from prison through the Reentry Community Services Program which aids reintegration into the community with support.

Project for Assistance in Transition from Homelessness (PATH)

We have designed these services to help identify, assess, and ensure the safety of those who are homeless and mentally ill. It is our goal to help link clients with services & treatment options that will address their immediate needs.

PATHFINDER

This service is a part of our PATH program and helps people who are experiencing an opioid and or stimulant use disorder get connected with services. This program is conducted by Peer SUD Specialists who have lived the experience and bring a personal perspective to their work.

Prosecutorial Diversion Program

This program identifies individuals detained in local jails with misdemeanor, non-violent felony, and some violent felony charges, with behavioral health conditions or developmental disabilities related to their crime, and for whom a question of competency was raised (the capacity to understand the nature of the proceedings against him or her to assist in his or her own defense). The identified individuals who do not have exclusionary violent or sex offenses and who consent to participate in the program for up to one year are diverted into the program. Program-assigned care coordinators help participant’s access community behavioral health services and other resources. The program goals include providing intensive care coordination which facilitates access to needed mental health, substance use, or developmental disability treatment and services, while building community supports to reduce criminal justice system encounters and orders for competency services. “The core belief of the program is that by offering life-enhancing services, rather than disciplinary action, participants will be given the best chance of success for improving their lives, [while] at the same time maintaining accountability”.
